How fees work
Your fee is set on a sliding scale, and you see it in writing before your first session.
Insurance we work with
Balm is a training clinic. We are not in network with any insurance plan, and we do not bill Medicaid or Medicare. If you want to use your own plan, you pay the session fee and we give you a receipt to send to it. Plans differ, so ask yours before you start.
Out of network with all plans
No Medicaid or Medicare billing
If you pay yourself
The sliding scale sets your fee for each session. At intake we ask about your household's size and income, and the scale gives you a fee that fits. A limited number of reduced-fee spots are open at a time; ask how to request one. Sessions are 50 minutes for individuals and 80 for couples. You pay by card at the end of each session, and HSA and FSA cards work. We ask for 24 hours' notice to change an appointment.
Receipts for reimbursement
Each month we give you an itemized receipt (a superbill) that you may send to your plan, which may reimburse part of an out-of-network visit. It carries the dates and type of session and a diagnosis code that your insurer can see. Your plan decides whether it pays anything.
An estimate in advance
If you pay without insurance, you have the right to receive a written Good Faith Estimate of your costs before you start. We give you one with your sliding-scale fee. If your final bill is $400 or more above the estimate, you can dispute it. The estimate is not a contract.
Checking your benefits
Because we do not bill your plan, we do not check your benefits. If you want reimbursement, call your plan before your first session and ask about out-of-network mental health benefits. The intake coordinator can tell you what to ask.

About your household
The number of people in your household and your approximate income. We use them to set your sliding-scale fee, and we do not ask for tax forms.
Your fee, in writing
You receive your fee before your first session. If your finances change, tell us and we will look at it together.
A way to pay
Most people pay by card at the end of each session. HSA and FSA cards work.

How do fees work?
Our sliding scale sets your fee from your household's size and income, and you have it in writing before your first session. A limited number of reduced-fee spots are open at a time. We review your fee with you if your finances change.
Can I use an HSA or FSA card?
Yes. Most health savings and flexible spending cards work for sessions. Check with your plan administrator if you are unsure what it covers.
What is your cancellation policy?
We ask for 24 hours' notice to change or cancel an appointment. If something unexpected happens, tell your associate and we will talk about it.
What is a superbill, and can I use it?
You may send your plan a superbill, an itemized receipt with the dates of your sessions and the type of service. Your plan may reimburse part of the fee. The receipt carries a diagnosis code that your insurer can see, and your plan decides what it pays.
